Here's an uncomfortable fact about the standard blood panel: your fasting glucose Blood Glucose (Fasting) and HbA1c Glycated Hemoglobin (HbA1c) can read "normal" for years while your metabolism quietly strains to keep them there. The number that reveals that hidden effort — fasting insulin — is rarely ordered. It may be one of the most useful early signals of how you're aging, and it's cheap to measure.
What fasting insulin actually tells you
Insulin is the hormone that moves sugar out of your blood and into your cells. When cells start resisting its signal, the body compensates by pumping out more insulin. For a long time that succeeds: blood sugar Blood Glucose (Fasting) stays normal — while insulin climbs in the background to make that happen. Measure fasting insulin (often combined with fasting glucose into a ratio called HOMA-IR) and you can see the strain before sugar ever budges.
The evidence: insulin moves years before glucose
The clearest picture comes from the Whitehall II study, which tracked 6,538 adults and looked backward from the moment of diabetes diagnosis. Insulin sensitivity didn't collapse overnight — it declined steeply across the five years before diagnosis, while fasting glucose only surged sharply in roughly the final three years. By the time a routine glucose or HbA1c Glycated Hemoglobin (HbA1c) test flags a problem, the process has often been building for half a decade.
And it isn't only about diabetes. Insulin resistance sits upstream of a cluster of issues — higher blood pressure, higher triglycerides Triglycerides and lower HDL High-Density Lipoprotein (HDL), more inflammation and abdominal fat — that together drive cardiovascular risk. Catching it early is a chance to change the trajectory, not just the diagnosis.
How to get it checked
Fasting insulin isn't exotic — it's just not default. Ask your doctor whether a fasting insulin test (ideally alongside fasting glucose, so HOMA-IR can be calculated) makes sense for you. It needs the same blood draw as the tests you already do.
Four habits that lower fasting insulin
The reason this marker is worth watching is that it responds — often quickly — to everyday behaviour.
- Walk after you eat. Moving muscles pull glucose from the blood without needing much insulin. A 2022 meta-analysis found short bouts of light walking significantly reduced both post-meal glucose and post-meal insulin versus sitting. A 2–10 minute walk after meals is one of the highest-return habits here.
- Protect your sleep. Insulin sensitivity is startlingly fragile to short sleep: in a controlled study, a single night of four hours' sleep cut insulin sensitivity by roughly 25% in healthy people. Consistent 7–9 hour nights are a lever, not a luxury.
- Build some muscle. Muscle is your largest sink for blood sugar. Around 30–60 minutes of muscle-strengthening a week was associated with 10–17% lower risk of death, cardiovascular disease and diabetes.
- Ease off fast carbohydrates. Refined carbs and sugary drinks demand the biggest, fastest insulin surges. Shifting toward whole grains, legumes, vegetables and protein — and eating them before the rice or noodles — flattens the spikes your pancreas has to answer.
An honest word on limits
Fasting insulin has no single universally-agreed "optimal" cutoff, and it should be read alongside your other markers, not alone.
